Our Agreement to Partner in your Health Care

Understanding Our Stand for your Health

At the MaxWell Clinic, we believe clarity in communication is a cornerstone of the partnership which exists between provider and patient in our clinic.  You will be asked to sign our “Consent for Treatment” when you become a patient.  Your signature indicates your agreement that you will be an active participant in the pursuit of wellness as prescribed by your MaxWell Clinic health care provider.  We ask that our patients read the principles in our “Consent for Treatment” document carefully and thoughtfully before signing it.

“Consent for Treatment”

I, ________________________ seek the health improvement, restoration and protection services of MaxWell Clinic physicians, practitioners, affiliates and staff.  The philosophy of care which underlies this practice is as follows:

1.   Health improvement and protection is best achieved in partnership between the participating patient and practitioner, affiliates and staff.   Trust, caring, mutual respect and understanding, are essential for this partnership.

2.    Health improvement and protection focuses on enhancing all dimensions of health – the physical, emotional, mental and spiritual rather than focusing on the physical body alone.

3.    A person’s lifestyle – including his or her diet, exercise patterns, sleep habits, stressors and interpersonal relationships – are directly related to the development and maintenance of health.  We strive to evaluate these factors and seek to help the patient understand and embrace positive behaviors regardless of age or type of medical problem.

4.    We seek to use what is WISE and WORKS for the benefit of our patients.  This includes all modalities that are Safe, Tolerable, Effective and Cost-effective.  We feel the practice of medicine must be focused upon the PATIENT, not bias towards one type of medical problem or treatment.

5.    Although prescription and over-the counter medications are used when the provider believes it is necessary, an attempt is first made to use products  that are natural to the body with primary focus on health production rather than on disease treatment. These include nutritional supplements such as vitamins, minerals, enzymes, amino acids, essential fatty acids and herbs.

6.   In addition to recommending that a patient take nutritional supplements orally, we may recommend that a patient receive a series of injections either intravenously or intramuscularly.  Two important reasons for recommending this procedure include:

a. The assurance that the particular substance gets into the body (which may not happen when the supplement is taken orally and the patient has absorption problems).

b.  To achieve a concentration of the substance in the bloodstream that may be difficult to achieve if the substance is taken by mouth.

Our Practice Is Exclusively An Office-Based Practice. We do not work in the hospital.  If you require hospitalization we will refer you to a hospital based specialist to co-ordinate your hospital stay.   If you are one of our out-of-state or international patients, we strongly recommend that in addition to our care you maintain a relationship with one or more local physicians appropriate to your condition and situation.
